Hey Tony,
What kind of lighting do you do for these shoots? How do you monitor audio when its SO loud?

Some of the venues have sufficient lighting. Especially this one down the street from me. They've got dozens of par cans and they just installed a bunch of LED lights. I also have 2 trees of 4 Par 38 cans and a few clamp lights I carry with me.
For audio. I grab a mix from the board, run my own mics and record the room, or just throw pre-recorded audio over it and sync it up. In this video above, I used the raw audio from the 3 cameras and compressed it a little bit and did my best to get it to sound ok. I try to check the mic levels during the band's sound check to make sure the mic levels aren't peaking too hard.
I have some decent headphones for monitoring audio, but most of the time the sound guys are on top of it. It's smart to get in good with all the sound guys at all the venues you do video shoots. They'll start to recognize you and are willing to go a little step farther to help you out if you explain what you're doing.
